You should also consult the links providing the checklist, relevant regulations, and rules of procedure for filing your eligibility appeal. Links to a sample appeal petition and certificate of service are provided for you to either (a) print out and fill in the blanks by hand (please be sure to attach the relevant attachments referred to therein); or (b) fill in the blanks on the computer and print out.
