Starting a health related business is not like running other business. It is far more complicated as it deals with a sensitive area. If I were in your place. I would have consulted with an attorney so that I will not have to face any legal complication in future. Better to be aware of the legal issues before you start headlong.

I think a subchapter s corp is better than an llc, but either way, you want to get the liability protection. I would definately consult a good lawyer on this, and I agree you need to look into the licensing requirements as well. Good luck!

Ultimately it is your decision. The incorporation status gets filed on a State level, and TaxID is on a federal. Don't forget to get an EIN (otherwise known as TaxID) and make sure that your state TaxID (NY is a great example of that) is separate from Federal EIN.

An LLC would work fine in this situation but I would consult with a local attorney to figure out which is best for tax reasons. The main thing you are going to run into is the large amount of licenses and certifications needed for this type of business.
Sure, however being in the health field requires many different types of licenses prior to actually doing business; and I'm not sure if you can actually do health care out of your own home. Your best bet is to contact your local county clerks office and\or business affairs division of your city and state and seek what proper licences are requirements have to be met to operate this kind of business.
