We closed an S-Corp in 2008 but would like to reinstate it. I have figured out how to reinstate it in our local state (Indiana), but can't figure out how to reinstate it Federally. Also, after reinstatement, we'd like to modify the name of the S-Corp.
Any help with correct forms would be greatly apprecated.

When you say 'reinstate it Federally,' do you mean with the IRS? If so, your accountant can help you. Business names and entity types are registered at the state level, not the federal level, so reinstating the S-corp in Indiana would be all you need to do to conduct business.
