Setup a health insurance plan for a startup

What I need to know:
1. What steps are involved in setting up a company health insurance plan from start to end?
2. Given our small size, should we do this in-house or outsource this to a firm that handles benefits?
3. Are benefits usually given to employees for free or are they subsidized?
4. What insurance providers or outsource firms are well known and have a good reputation?
5. Is there anything else that I should know or look out for?

You are probably long past this with your business. But I can't help but jump
in here to give you some quick advise. You have a business to run, so don't
waste your time trying to figure out insurance. Call an insurance broker
(some one that is independent) and have them help you through the details.
I do not suggest that you call an insurance carrier. They will send you one
of their agents and he/she will give you only one option. Call an insurance
broker and they will make your life easy!
