Important HUBZone Document Request for Corporations

This form is for personal business use and is not sent to the SBA HUBZone program. The documents listed in the form are used to determine eligibility along with the information provided via the online application. This document can also be used as a checklist to assist one in gathering supporting documents to submit to the SBA HUBZone Program Office. Please note that depending on  particular circumstances, an applicant may be asked to submit additional documents that are not listed on this form.
