Application Forms:

• Small Business Application (SBA Form 2301-A) Applicant completes pages 1 and 2; the remaining pages are instructions and statements that need to be read by the applicant.
• Application for Guaranty (SBA Form 2301-B) Lender completes page 1; the remaining pages are instructions. Include a credit memorandum that meets reasonable and prudent industry standards. (See page 5 for details).
• Eligibility Questionnaire (SBA Form 2301-C) Lender completes this questionnaire. A false answer indicates further analysis is required, but does not mean that the loan is ineligible.
• Request for Copy or Transcript of Tax Form (Form 4506-T) Copy to SBA with date submitted to IRS.

Loan packages should be sent to the 7(a) Loan Guaranty Processing Center:

• Online. It is easy to use, does not have file size limitations, and can be accessed by several SBA personnel.
• By email to 7aloanprogram@sba.gov - as long as the total file size is less than 9 MB. If the file size nears or exceeds 9 MB, the email will not go through and neither the sender not SBA will receive notification.
• By mail or FedEx to 6501 Sylvan Road, Suite 122, Citrus Heights, CA 95610
• By Fax to 916-735-1975
