Updated 504 Forms 1244, 2450, 2234 (Part C)

This Notice announces revisions that SBA has made to certain forms used in the 504 Loan Program. The revisions are organizational in nature, and do not affect the substance of the information currently collected. Program Participants may begin using the revised forms immediately.

Summary of Changes:

SBA Form 1244, Application for Section 504 Loan

• Page 1, Part A - Modified the citation in Exhibit 3 to refer to Section XIX, the new location of the questions formerly located in Section II.B.

• Page 7, Part B - Relocated the questions formerly located in Section II.B, which are required to be completed by all Subject Individuals considered to be “Associates” of the applicant, to new Part B, Section XIX (Page 7), adding fields to identify the Subject Individual completing the section and his or her association with the Applicant. Subject Individuals must sign to certify their answers on Section XIX, Page 7. If the Subject Individual is a Proprietor, General Partner, Limited Partner or Stockholder owning 20% or more, or Guarantor, he/she must also sign the application on page 11.

• Page 11, Part C - Relocated “Borrower Agreements and Certifications” to Applicant signature page.

• Page 12, Part C - Relocated “CDC Agreements and Certifications” to CDC signature page.

• Page 5, Part VII – Amended cross-reference to SBA Form 1244 to reflect new Section XIX.

• Page 7, Part VIII – Amended cross-reference to SBA Form 1244 to reflect new Section XIX.
